Membership Applications

GreenCross of NZ will be open to support all that present a bona fide need for medicinal cannabis on the advice of their physician or specialist.

If you wish to apply to become a full member and have or can obtain the backing of a registered medical practitioner please download the full member application forms in MS WORD format. This is a two page document the first to be filled in by the patient and the second to be filled in by a doctor/specialist.
